REGISTRATION

Our registration process is very simple when we are able to take on new patient. Please contact us to confirm availability. All we require prior to your first consultation is for you to complete a Medical History and Registration Form which can be collected from our surgery or downloaded at the following link – Medical History Form. Please ensure you provide a full detailed list of all your medication.

PAYMENTS

At your first appointment, you will be expected to pay for your basic examination charge. Any further treatment that you may require and have requested will be discussed with you and you will be given an estimate prior to any charges. Payment for treatment will then be collected subsequently over the course of treatment – you will be informed how much is due each visit depending on your treatment plan.

We accept cash, and credit and debit cards except for American Express.
